  • Height and Weight: 5'7, 130-150 pounds
  • Distinguishing Characteristics: Light brown hair, blue eyes. Cook's left ear is pierced. He has a tattoo of a lizard and a tree on his back, a tattoo of three hearts with the names "Robert," "Cody," and "Tim" on the right side of his chest, a tattoo of a woman and name "Crystal" on the left side of his chest, and unspecified tattoos on his forearms. Cook also has scars on the bridge of his nose and on his hand. His hair was cut in military style, very short, at the time of his disappearance.

    Details of Disappearance
    Cook was last seen at his residence in the vicinity of the 500 block of Kirby Street in Shreveport, Louisiana at about 9:00 p.m. on May 21, 2003. He left his home, driving his red 1981 GMC pickup truck with Louisiana license plates numbered W106450. He said he was going to see a man to purchase a computer. He never showed up and neither he nor his vehicle have ever been seen again. Cook was last seen wearing dark blue pants, black boots, and a gold wedding band.
    Cook may have been seen at a local casino that evening but this sighting has not been confirmed. His wife reported him missing at 12:00 p.m. on May 22. Investigators say Cook disappeared under suspicious circumstances. His case is unsolved.
